- Learn about your health
- Get ready to leave the hospital

You can join or stop the program anytime. It’s your choice.
Programs We Offer
We have different Case Management programs to help with different needs.
Maternity Case Management – For pregnant members. We help you before and after your baby is born. We make sure you get the care you need.
Care Coordination – Helps you connect all the care you need. If you see many doctors or take a lot of medicine, we help make sure everything works together.
